Buddy Information Form:  

This form must be filled out by girls 16 to 18 years of age who are attending camp as a buddy to a camper and that camper’s parent/guardian.  This form should be attached to the buddy’s Signature Form.

Busing Form:

This form is for leaders only and should be printed and brought to the busing location on busing day.  Stake leaders must fill out this form and turn it in at the Open Enrollment table by 8:20 am.

Early Release Form:

This form should ONLY be filled out if you are leaving camp early.  Please understand that campers who leave camp early will not be permitted to return.

Epipen Form:

Campers bringing an epipen to camp MUST fill out this form and turn it in to their stake leader as part of the registration process.  Open Enrollment campers or campers from participating stakes who are registering late should mail this form to Heather McKenna, 7502 Park Maple Dr, West Jordan UT 84081.  Forms can also be scanned and emailed to oakcrestheather@gmail.com.

New Medical Form:

Any additional medical information or changes occurring after registration has been completed online can be indicated by filling out this form and turning it in on busing day to your stake leader or to the Open Enrollment table at the busing location.

Projection Questionnaire:

This form is for leaders only.  It should be printed, filled out, and turned in at the Open Enrollment table on busing day.  The Oakcrest Women’s Committee uses this form to determine camp date assignments for the following year.
